Talking Points:

Talking Points (formerly UDSET) has been developed by the Joint Improvement Team to support a focus on the outcomes important to users of community care services in Scotland, with a related version (CDSET) for unpaid carers. Outcomes in this context are understood both as the goals that users and carers want to achieve in partnership with services, and as the impact of services on individual lives.

Pilot Project Plan Outline

Objectives:

The overall objective of this pilot is to find out whether the use by Care Home staff of a Communication Form will improve the quality of calls to NHS 24. Will it:

  • improve communication between care home staff and NHS 24?
  • allow a more informed initial assessment of the resident in the care home?
  • maximise the opportunity for a resident to receive healthcare advice and treatment within the care home setting?
  • improve access to the appropriate unscheduled care services?
  • improve partnership working between care homes and NHS 24?
