In Tayside, Connect in Care is working with two separate groups on two different projects:

 

1. Managing the Life Transition that Follows an Early Diagnosis of Dementia

Tayside Y1 Project

The objective of this project is to see to what extent the development of counselling skills among CPNs will impact on the relationship-based care they provide to support individuals and their families following an early diagnosis of dementia. A team of CPNs are taking a (funded) counselling course through the academic year 2007-08. They are recording their Practice Development over a one year period.

 

 

2. Managing Transitions between Care Home and Hospital

Tayside Y1/Y2 Project

The objective of this project is to share experience and expertise across sectors to better support the health and well-being of older people with confusion during transition from care home to hospital and between hospital wards. We are working with staff from care homes, acute and community (CHP) sectors.

We are about to trial a method that the group has developed. The results will be shared when collected.
